For many years, physicians viewed diabetes and dementia as two separate health issues. Diabetes was primarily considered a blood sugar disorder that could damage organs like the heart, kidneys, and eyes. Dementia, meanwhile, was seen as a decline in memory and thinking abilities originating in the brain. However, current research suggests these two conditions are more interconnected than previously believed.

Studies worldwide are revealing that blood sugar regulation can impact brain health, and changes within the brain might also influence how the body manages glucose. This growing body of evidence indicates that maintaining metabolic health could be crucial for preserving brain function as we age.

Diabetes rates are rising globally, with hundreds of millions of individuals living with the condition, most of whom have type 2 diabetes. In this form, the body becomes increasingly resistant to insulin, a hormone that facilitates the entry of glucose (sugar) into cells for energy. When insulin doesn’t work correctly, sugar builds up in the bloodstream, gradually harming various tissues throughout the body.

Dementia, also a major health concern, encompasses multiple conditions that impair memory, cognitive skills, language, and daily functioning. Alzheimer’s disease is the most common form. Since both diabetes and dementia become more frequent with age, researchers have explored whether they share common biological pathways.

Evidence now indicates that those with diabetes have approximately a 60% higher risk of developing dementia compared to individuals without diabetes. Additionally, repeated episodes of hypoglycemia (low blood sugar) have been linked to an increased risk of memory issues and cognitive decline.

One possible reason for this link is insulin resistance in the brain. Although the brain accounts for just a small fraction of total body weight, it consumes about 20% of the body’s energy. Brain cells heavily depend on glucose for functions like memory storage, movement control, and information processing. In Alzheimer’s disease, these cells often have trouble using glucose effectively; they become less responsive to insulin and struggle to produce sufficient energy. Because this resembles some aspects of type 2 diabetes, Alzheimer’s is sometimes referred to as “type 3 diabetes.”

The relationship between diabetes and dementia appears to be bidirectional. Research shows that individuals with Alzheimer’s often have higher fasting blood sugar levels even if they are not diabetic. Animal studies have demonstrated that Alzheimer’s-related brain changes can elevate blood glucose, and carriers of the APOE4 gene—a strong genetic risk factor for Alzheimer’s—often exhibit reduced insulin sensitivity.

Blood vessel damage presents another key link. Diabetes can harm blood vessels throughout the body, including those in the brain. Chronic high blood sugar levels and wide fluctuations can damage tiny vessels that supply the brain, leading to decreased blood flow, less oxygen, and fewer nutrients reaching brain tissue. Additionally, diabetes can impair the blood-brain barrier, the protective system that keeps harmful substances out of the brain. When this barrier is compromised, inflammation can develop, which researchers increasingly believe contributes to dementia progression.

This growing understanding has prompted scientists to explore whether diabetes medications might also support brain health. Some medications initially designed for diabetes show promise. For example, memantine, used for moderate to severe Alzheimer’s, was originally developed as a diabetes drug. While it didn’t succeed as a blood sugar treatment, it was found to improve certain cognitive functions. Conversely, metformin, the most prescribed drug for type 2 diabetes, appears to reduce brain inflammation, and some evidence suggests that diabetics on metformin have a lower risk of dementia.

Newer treatments are also under investigation. GLP-1 receptor agonists like Ozempic and Wegovy, which lower blood sugar and aid weight loss, have been linked to a decreased risk of dementia in some studies. Large-scale clinical trials are underway to determine if these drugs can slow cognitive decline in early Alzheimer’s cases. Similarly, SGLT2 inhibitors, which remove excess glucose via urine, are showing potential in reducing brain inflammation and lowering dementia risk.

Innovative approaches, such as delivering insulin directly to the brain through nasal sprays, are being explored too. Early studies hint at potential benefits for memory and brain volume, but more extensive research is required.

Ultimately, the consensus among scientists is that managing blood sugar effectively may be vital for brain health. Many shared biological pathways—like insulin resistance, inflammation, blood vessel damage, and cellular energy issues—tie these conditions together. While several questions remain, these insights foster hope that improving diabetes management could play a significant role in preventing or delaying dementia. Moreover, existing diabetes treatments might eventually be repurposed to protect the aging brain.
