New Paint-On Solution Promises Relief from Tooth Decay

Tooth decay remains the most common chronic disease among children, yet many young kids go without treatment until their cavities worsen significantly.

Every year, thousands of children in the U.S. visit emergency rooms due to intense tooth pain and infections.

A recent study from the University of Michigan suggests that a straightforward liquid treatment could prevent many of these painful episodes. The study appeared in JAMA Pediatrics.

Most young children find dental drills and injections intimidating. Some require treatment under general anesthesia because they’re too young to tolerate traditional procedures, which adds risks, costs, and longer wait times for families.

The treatment examined is called silver diamine fluoride, or SDF. Dentists apply a clear liquid directly onto cavities using a small sponge—taking only seconds, with no drilling or injections, and usually little discomfort.

Used successfully in numerous countries for decades and available in the U.S. since 2014 to treat tooth sensitivity, SDF has also been used to arrest cavities. However, it’s considered off-label in the U.S. since large-scale clinical trials weren’t available until now.

To generate stronger evidence, researchers conducted a Phase III clinical trial with 830 children under age six, recruiting participants from dental clinics, pediatric practices, and Head Start programs in Michigan, New York, and Iowa.

The results showed that 38% of SDF treatments successfully halted decay in more than half of the affected primary teeth after reapplication every six months. This indicates many cavities stopped progressing without needing fillings or more invasive procedures.

Lead researcher Professor Margherita Fontana explained the treatment was both effective and safe—even for children as young as one year old. This is crucial because early childhood tooth decay can lead to pain, infections, eating difficulties, sleep disturbances, missed school, and repeated medical visits.

Researchers also see potential for SDF to benefit older adults, individuals with disabilities, and patients with severe dental anxiety who struggle to access traditional dental care. For many, it could slow or prevent decay while awaiting definitive treatment—or avoid it altogether.

A key drawback is cosmetic: the affected tooth areas turn permanently black after treatment. Nonetheless, many families might accept this trade-off if it spares children pain, infections, hospital visits, or surgery.

This significant U.S. clinical trial offers the strongest evidence yet that silver diamine fluoride is a safe, effective method for stopping cavities in young children. The findings could pave the way for FDA approval and promote broader adoption by dentists, pediatricians, and insurance providers.

Although the discoloration is a concern, the treatment provides an affordable, practical solution that could markedly improve children’s dental health and reduce unnecessary surgical interventions.
