The foods people consume daily may have a more significant impact on digestive health than many realize. An extensive international study has revealed that consuming high levels of ultraprocessed grain products could raise the risk of developing inflammatory bowel disease (IBD), a serious condition affecting millions worldwide. These findings were published in the American Journal of Gastroenterology and add to a growing body of evidence linking highly processed foods to chronic health issues. Researchers suggest that this information could influence future dietary guidelines aimed at preventing digestive disorders.

Inflammatory bowel disease refers to long-lasting conditions characterized by inflammation in the digestive tract. The most common types are Crohn’s disease and ulcerative colitis. Symptoms often include abdominal pain, diarrhea, bleeding, fatigue, weight loss, and nutritional deficiencies. Severe cases might require surgical intervention. While the exact causes remain unclear, experts believe factors like genetics, immune responses, environmental exposures, and lifestyle choices all play a role. Among these, diet has become a key focus of research.

Modern diets look quite different from those of previous generations, largely due to the popularity of packaged foods that are quick, affordable, and easily accessible. These foods frequently contain numerous added ingredients designed to enhance flavor, texture, or shelf life. These products are often classified as ultraprocessed foods, which include snack foods, sweetened cereals, commercial baked goods, processed breads, crackers, and grain-based products that undergo extensive industrial processing.

Researchers analyzed dietary data from the Prospective Urban Rural Epidemiology (PURE) study, a large-scale international project involving participants from around the globe. The study examined dietary habits of 124,590 adults from 21 countries, using detailed food questionnaires to estimate consumption of ultraprocessed grain products.

Participants were followed over time to track the development of IBD. By comparing their diets with health outcomes, researchers identified clear associations: higher intake of ultraprocessed grains correlated with increased risk of IBD. Specifically, individuals eating at least 19 grams of ultraprocessed grains daily faced a significantly higher risk than those consuming less than 9 grams.

Furthermore, heavy consumption of ultraprocessed foods overall was linked to an even greater risk. People who ate five or more servings of these foods daily nearly quadrupled their chance of developing IBD compared to those consuming fewer than one serving a day. Interestingly, less processed grains like fresh bread and rice were associated with a decreased risk, indicating that processing might be a key factor rather than the grains themselves.

Several biological mechanisms could explain these findings. The trillions of microorganisms in our gut, collectively known as the microbiome, are essential for digestion, immunity, and overall health. Food additives and processing techniques may disrupt this delicate microbial balance, leading to increased inflammation and weakening the gut’s protective lining. Over time, this could contribute to the development of IBD.

Another theory is that highly processed foods might provoke chronic low-grade inflammation throughout the body—a condition linked to obesity, diabetes, heart disease, and some cancers. The rising prevalence of IBD globally, including in developing countries experiencing rapid dietary shifts, underscores the importance of these findings.

While the study presents compelling associations, it’s important to emphasize that it does not prove causality. Observational research identifies correlations but cannot confirm that ultraprocessed grains directly cause IBD. There may be other lifestyle factors at play. Some of the researchers also disclosed relationships with pharmaceutical and nutrition companies, a common practice to maintain transparency.

Despite these limitations, the study’s large scale, diverse population, and long-term follow-up make the findings highly valuable. They reinforce the advice to favor minimally processed foods and limit heavily processed options for better digestive health. Future research will help clarify how these foods influence the gut and whether reducing their intake can actively lower disease risk.

Meanwhile, public health efforts encourage choosing whole, unprocessed grains and fresh foods.
